Sometimes Care Begins With One Meaningful Moment.

A flower. A visit. A word of encouragement. A connection to someone who can help.

Care Pathways brings together the compassionate care and outreach work of Helpmate Ministries, creating meaningful ways for people to feel seen, valued, encouraged, and remembered.

Small Acts Can Create Meaningful Moments

Care Pathways reflects a simple belief: people should not have to move through difficult seasons feeling forgotten or alone.

Through compassionate outreach, acts of kindness, and developing connections to professional support, Helpmate Ministries creates pathways that encourage dignity, connection, and care.

THE FORGOTTEN ONES

Helping Residents Feel Seen, Valued, and Remembered

The Forgotten Ones is our nursing home outreach initiative focused on residents who may receive few or no visitors.

Through flowers, thoughtful gifts, and meaningful personal connection, we create moments that remind residents they have not been forgotten.

THE JOY IN BLOOM PROJECT

Sharing Encouragement, One Bloom at a Time

The Joy in Bloom Project brings flowers and encouraging moments into public and community spaces.

Each bloom becomes a simple reminder that kindness can meet someone in an ordinary moment and brighten the path ahead.

CARE CONNECTION

Care Connection is a developing referral pathway intended to help eligible low-income individuals, couples, and families connect with licensed counselors.
